Referrals

Have you been referred to us?

Many of the patients we see are referred to us. Please see below the various referral pathways that we accept to assist you in claiming your rebate or accessing our services. Otherwise, feel welcomed to give our friendly team a call on (02) 5301 6585.

Medicare

Many of our patients are referred by their doctor. Often the GP will allocate a number of sessions (max of 5) on an EPC plan. This entitles the patient to receive a significant rebate on the cost of treatment. As well as EPC referrals, GP’s may refer further sessions under Indigenous (max of 5) or Type 2 diabetes group sessions (8 sessions). We are able to process your rebate on the spot, which saves you going to a Medicare Office to submit your claim.

DVA

The Department of Veterans Affairs (DVA) covers the cost of their veterans with white (specific condition only) or gold cards for “clinically necessary treatment”. GPs will refer such patients on a D0904 referral. After the initial consultation, our AEP’s will create a veteran care plan to submit to your GP to outline the intended treatment and reasons for undertaking such treatment. We obtain payment through the DVA for such services and the veteran has no out of pocket expense.

WorkCover/CTP

Under the WorkCover NSW or CTP schemes, injured patients at work or motor vehicle accidents will have a case manager and nominated treating doctor. Either of these people may refer you for functional work conditioning or active injury rehabilitation. In most instances, all the formalities of the referral and payment will be dealt with by communication with the third party insuring body. The patient bares no out of pocket expense on a WorkCover or CTP referral.

NDIS

The National Disability Insurance Scheme (NDIS) is the new way that those people with disabilities can manage their individual needs by allocating funding to specific areas that will improve their quality of life. Our AEPs love to assist people achieve greater health, improved daily living and maintenance of independence.

Whether you, a carer or a third party are managing your specific needs for your disability, give our team a call on (02) 5301 6585 to find out the positive impact of having an AEP in your NDIS plan will give to your life.

Private

Many of our patients are referred to us by their family or friends who have experienced great outcomes whilst working with us. We love hearing from such patients, as they make us work even harder to continue to assist our patients to achieve their best health possible!

We are registered with private health insurers, so you may be eligible for rebates depending on your level of cover.
